A giant Müllerian duct cyst in the perineum: a case report

The authors present a case report of a giant Müllerian duct cyst in the perineum. A 37-year-old man presented with a mass with the size of 50 cm×40 cm×30 cm in the perineum. Computed tomography (CT) scan and magnetic resonance imaging (MRI) of the pelvis and perineum found a huge multilocular cystic mass. X-ray film of the pelvis showed a giant soft-tissue shadow of high density in the perineum. Voiding cystourethrogram did not show any cyst interlinked to posterior urethra. After related examinations, the patient underwent open surgical resection of the cyst. Pathologic examination confirmed the presence of a Müllerian duct cyst. Huge Müllerian duct cyst is uncommon in clinic. Treatments of Müllerian duct cysts depend on their sizes and symptoms.
Bile duct injury repair — earlier is not better

Bile duct injury is a common complication of cholecystectomy. The timing of bile duct injury repair remains controversial. A recent review conducted in France reported 39% complications and 64% failure after immediate repair in 194 patients compared with 14% complications and 8% failure after late repair in 133 patients. A national review of 139 consecutive early repairs conducted at five hepatopancreaticobiliary centers in Denmark reported 4% mortality, 36% morbidity, and 42 restrictures (30%) at a median follow-up of 102 months, and only 64 patients (46%) demonstrated uneventful short-term and long-term outcomes. Most patients with bile duct injury present with bile leak and sepsis; thus, early repair is not recommended. Percutaneous drainage of bile and endoscopic stenting are the mainstays of treatment of bile leak because they convert acute bile duct injury into a controlled external biliary fistula. The ensuing benign biliary stricture should be repaired by a biliary surgeon after a delay of 4–6 weeks once the external biliary fistula has closed.

Strategies of minimally invasive treatment for intrahepatic and extrahepatic bile duct stones

Cholelithiasis is a kind of common and multiple diseases. In recent years, traditional laparotomy has been challenged by a minimally invasive surgery. Through literature review, the therapeutic method, effect, and complications of minimally invasive treatment of intrahepatic and extrahepatic bile duct stones by combining our practical experience were summarized as follows. (1) For intrahepatic bile duct stones, the operation may be selected by laparoscopic liver resection, laparoscopic common bile duct exploration (LCBDE), or percutaneous transhepatic cholangioscopy. (2) For concomitant gallstones and common bile duct stones, the surgical approach can be selected as follows: laparoscopic cholecystectomy (LC) combined with endoscopic sphincterotomy (EST) or endoscopic papillary balloon dilatation, LC plus laparoscopic transcystic common bile duct exploration, LC plus LCBDE, and T-tube drainage or primary suture. (3) For concomitant intrahepatic and extrahepatic bile duct stones, laparoscopic liver resection, choledochoscopy through the hepatic duct orifice on the hepatectomy cross section, LCBDE, EST, and percutaneous transhepatic cholangioscopic lithotripsy could be used. According to the abovementioned principle, the minimally invasive treatment approach combined with the surgical technique and equipment condition will be significant in improving the therapeutic effect and avoiding the postoperative complications or hidden dangers of intrahepatic and extrahepatic bile duct stones.

Diffuse cystic lung diseases are uncommon but can present a diagnostic challenge because increasing number of diseases have been associated with this presentation. Cyst in the lung is defined as a round parenchymal lucency with a well-defined thin wall (<2 mm thickness). Focal or multifocal cystic lesions include blebs, bullae, pneumatoceles, congenital cystic lesions, traumatic lesions, and several infectious processes such as coccidioidomycosis, Pneumocystis jiroveci pneumonia, and hydatid disease. “Diffuse” distribution in the lung implies involvement of all lobes. Diffuse lung involvement with cystic lesions can be seen in pulmonary lymphangioleiomyomatosis, pulmonary Langerhans’ cell histiocytosis, lymphoid interstitial pneumonia, Birt-Hogg-Dubé syndrome, amyloidosis, light chain deposition disease, honeycomb lung associated with advanced fibrosis, and several other rare causes including metastatic disease. High-resolution computed tomography of the chest helps define morphologic features of the lung lesions as well as their distribution and associated features such as intrathoracic lymphadenopathy. Correlating the tempo of the disease process and clinical context with chest imaging findings serve as important clues to defining the underlying nature of the cystic lung disease and guide diagnostic evaluation as well as management.

采用蒙特卡罗程序MCNP模拟计算了医院中子照射器I型堆(IHNI-1)热中子束流孔道出口处的等效平面源。对B堆芯进行了临界搜索计算,模拟计算了热中子束流孔道及出口处中子、γ的束流参数,应用等效平面源模型建立了BNCT等效中子、γ平面源。为人体头颅等效模型剂量分布的快速计算提供了较为可靠的平面源。

大豆胞囊线虫(SCN,Heterodera glycines)是危害全球大豆生产最严重的线虫。作物轮作和培育抗线虫品种是防治线虫的主要方法。随着越来越多能够克服常用抗源的毒性SCN 群体出现,有必要寻找更多新的抗源或拓宽抗性遗传背景。本文综述了大豆抗SCN 基因的研究进展,以及这些基因如何相互作用,从而使大豆能够抗SCN。本文还提供了最新分子作图和分子标记,可用于不同抗性品系和品种的大规模选择和鉴定,以加速常规育种计划。深入了解SCN 寄生蛋白和大豆对病原体的抗性是通过基因修饰、基因叠加、回交或基因工程引入新的抗源而使抗源多样化的关键。

The kinematic assumptions upon which the Euler-Bernoulli beam theory is founded allow it to be extended to more advanced analysis. Simple superposition allows for three-dimensional transverse loading. Using alternative constitutive equations can allow for viscoelastic or plastic beam deformation. Euler-Bernoulli beam theory can also be extended to the analysis of curved beams, beam buckling, composite beams and geometrically nonlinear beam deflection. In this study, solving the nonlinear differential equation governing the calculation of the large rotation deviation of the beam (or column) has been discussed. Previously to calculate the rotational deviation of the beam, the assumption is made that the angular deviation of the beam is small. By considering the small slope in the linearization of the governing differential equation, the solving is easy. The result of this simplification in some cases will lead to an excessive error. In this paper nonlinear differential equations governing on this system are solved analytically by Akbari-Ganji’s method (AGM). Moreover, in AGM by solving a set of algebraic equations, complicated nonlinear equations can easily be solved and without any mathematical operations such as integration solving. The solution of the problem can be obtained very simply and easily. Furthermore, to enhance the accuracy of the results, the Taylor expansion is not needed in most cases via AGM manner. Also, comparisons are made between AGM and numerical method (Runge-Kutta 4th). The results reveal that this method is very effective and simple, and can be applied for other nonlinear problems.

具有四方ThMn12型结构的富铁化合物有潜力满足当下人们对于高磁能积、工作温度为150~200 ℃的贫稀土永磁体的需求。尽管磁体制备技术的发展滞后于相关磁性材料的研究是正常的,但对于ThMn12型磁性材料而言,这种研究进展的差异非常显著。近年来,随着以含少量结构稳定元素(如SmFe11V或Sm0.8Zr0.2Fe9.2Co2.3Ti0.5)或者不含该类元素的材料(如SmFe9.6Co2.4薄膜)为基础所合成的具有优异内禀磁性化合物研究的突破,这一差距进一步扩大。在日益强大的理论计算的帮助下,人们对于理想的化合物的探寻从未停步。遗憾的是,基于聚合物键合填隙改性粉末的磁体的研究仍然处于边缘阶段。人们发现,引入镧(La)可以提高Sm(Fe,Ti)12中少数低熔点相的稳定性,从而首次使得液相烧结成为可能。然而,La金属的高反应活性会明显地破坏材料的矫顽力(Hc)。最初被抑制的ThMn12型相的可控结晶使得“块状”磁硬化成为可能,这不仅在Sm-Fe-V合金中可以实现(自20世纪90年代便为人所知),并且在添加La的(Ce,Sm)(Fe,Ti)12合金中也可以实现。然而,块状硬化所得的合金的性能仍然不能令人满意。机械化学合成的(Sm,Zr)(Fe,Si)12和(Sm,Zr)(Fe,Co,Ti)12粉末可能适合烧结成具有高强度的全致密磁体,尽管在此之前,两者都已开发出较高的各向异性,而后一种合金已开发出较高的矫顽力。
