A giant Müllerian duct cyst in the perineum: a case report

The authors present a case report of a giant Müllerian duct cyst in the perineum. A 37-year-old man presented with a mass with the size of 50 cm×40 cm×30 cm in the perineum. Computed tomography (CT) scan and magnetic resonance imaging (MRI) of the pelvis and perineum found a huge multilocular cystic mass. X-ray film of the pelvis showed a giant soft-tissue shadow of high density in the perineum. Voiding cystourethrogram did not show any cyst interlinked to posterior urethra. After related examinations, the patient underwent open surgical resection of the cyst. Pathologic examination confirmed the presence of a Müllerian duct cyst. Huge Müllerian duct cyst is uncommon in clinic. Treatments of Müllerian duct cysts depend on their sizes and symptoms.

Bile duct injury repair — earlier is not better

Bile duct injury is a common complication of cholecystectomy. The timing of bile duct injury repair remains controversial. A recent review conducted in France reported 39% complications and 64% failure after immediate repair in 194 patients compared with 14% complications and 8% failure after late repair in 133 patients. A national review of 139 consecutive early repairs conducted at five hepatopancreaticobiliary centers in Denmark reported 4% mortality, 36% morbidity, and 42 restrictures (30%) at a median follow-up of 102 months, and only 64 patients (46%) demonstrated uneventful short-term and long-term outcomes. Most patients with bile duct injury present with bile leak and sepsis; thus, early repair is not recommended. Percutaneous drainage of bile and endoscopic stenting are the mainstays of treatment of bile leak because they convert acute bile duct injury into a controlled external biliary fistula. The ensuing benign biliary stricture should be repaired by a biliary surgeon after a delay of 4–6 weeks once the external biliary fistula has closed.

Exergy-energy analysis of full repowering of a steam power plant

摘要: A 320 MW old steam power plant has been chosen for repowering in this paper. Considering the technical conditions and working life of the power plant, the full repowering method has been selected from different repowering methods. The power plant repowering has been analyzed for three different feed water flow rates: a flow rate equal to the flow rate at the condenser exit in the original plant when it works at nominal load, a flow rate at maximum load, and a flow rate when all the extractions are blocked. For each flow rates, two types of gas turbines have been examined: V94.2 and V94.3A. The effect of a duct burner has then been investigated in each of the above six cases. Steam is produced by a double-pressure heat recovery steam generator (HRSG) with reheat which obtains its required heat from the exhaust gases coming from the gas turbines. The results obtained from modeling and analyzing the energy-exergy of the original steam power plant and the repowered power plant indicate that the maximum efficiency of the repowered power plant is 52.04%. This maximum efficiency occurs when utilizing two V94.3A gas turbines without duct burner in the steam flow rate of the nominal load.

Diagnosis and treatment of thyroglossal duct carcinoma: Report of three cases with review of literatures

摘要: Thyroglossal duct carcinoma, which is usually diagnosed postoperatively, is a rare malignant tumor arising in the thyroglossal duct cyst. The definitive diagnosis can be made only after microscopic examination. We retrospectively reviewed three cases of thyroglossal duct carcinoma diagnosed in Peking University School and Hospital of Stomatology from January 1986 to August 2006. Clinical and pathological features were investigated and the optimal treatment protocol was proposed. The constituent ratio of thyroglossal duct carcinoma among surgically excised thyroglossal duct lesions was 2.9%. The clinical presentation of thyroglossal duct carcinoma was very similar to that of its benign counterpart. Two cases were diagnosed as thyroglossal duct cyst prior to the operation, the remaining one as dermoid cyst. All three cases were diagnosed as papillary carcinoma of thyroid origin after microscopic examination. Primary thyroglossal duct carcinoma should conform to the following criteria: localization of the carcinoma to a clearly demonstrable thyroglossal duct cyst or tract; clinically or histologically confirmed absence of carcinoma of the thyroid gland. Papillary carcinoma is the most common histological type, which usually develops slowly with an excellent prognosis. The histological characteristics including: formation of papillary structure; nuclear morphological variations such as ground glass nuclei, pseudoinclusions, intranuclear grooves and filaments; concentrically calcified structures termed psammoma bodies which is regarded as a strong indication of papillary carcinoma; and positivity in immunohistological staining for thyroglobin. Sistrunk procedure of excision is the choice for treatment. A close follow-up is needed. In the presence of thyroid gland masses or cervical lymphadenopathy, thyroidectomy or neck dissection should be recommended. The effect of thyroid suppression therapy and radioactive iodine therapy is not conclusive.

Strategies of minimally invasive treatment for intrahepatic and extrahepatic bile duct stones

Cholelithiasis is a kind of common and multiple diseases. In recent years, traditional laparotomy has been challenged by a minimally invasive surgery. Through literature review, the therapeutic method, effect, and complications of minimally invasive treatment of intrahepatic and extrahepatic bile duct stones by combining our practical experience were summarized as follows. (1) For intrahepatic bile duct stones, the operation may be selected by laparoscopic liver resection, laparoscopic common bile duct exploration (LCBDE), or percutaneous transhepatic cholangioscopy. (2) For concomitant gallstones and common bile duct stones, the surgical approach can be selected as follows: laparoscopic cholecystectomy (LC) combined with endoscopic sphincterotomy (EST) or endoscopic papillary balloon dilatation, LC plus laparoscopic transcystic common bile duct exploration, LC plus LCBDE, and T-tube drainage or primary suture. (3) For concomitant intrahepatic and extrahepatic bile duct stones, laparoscopic liver resection, choledochoscopy through the hepatic duct orifice on the hepatectomy cross section, LCBDE, EST, and percutaneous transhepatic cholangioscopic lithotripsy could be used. According to the abovementioned principle, the minimally invasive treatment approach combined with the surgical technique and equipment condition will be significant in improving the therapeutic effect and avoiding the postoperative complications or hidden dangers of intrahepatic and extrahepatic bile duct stones.

Diffuse cystic lung diseases are uncommon but can present a diagnostic challenge because increasing number of diseases have been associated with this presentation. Cyst in the lung is defined as a round parenchymal lucency with a well-defined thin wall (<2 mm thickness). Focal or multifocal cystic lesions include blebs, bullae, pneumatoceles, congenital cystic lesions, traumatic lesions, and several infectious processes such as coccidioidomycosis, Pneumocystis jiroveci pneumonia, and hydatid disease. “Diffuse” distribution in the lung implies involvement of all lobes. Diffuse lung involvement with cystic lesions can be seen in pulmonary lymphangioleiomyomatosis, pulmonary Langerhans’ cell histiocytosis, lymphoid interstitial pneumonia, Birt-Hogg-Dubé syndrome, amyloidosis, light chain deposition disease, honeycomb lung associated with advanced fibrosis, and several other rare causes including metastatic disease. High-resolution computed tomography of the chest helps define morphologic features of the lung lesions as well as their distribution and associated features such as intrathoracic lymphadenopathy. Correlating the tempo of the disease process and clinical context with chest imaging findings serve as important clues to defining the underlying nature of the cystic lung disease and guide diagnostic evaluation as well as management.

采用蒙特卡罗程序MCNP模拟计算了医院中子照射器I型堆(IHNI-1)热中子束流孔道出口处的等效平面源。对B堆芯进行了临界搜索计算,模拟计算了热中子束流孔道及出口处中子、γ的束流参数,应用等效平面源模型建立了BNCT等效中子、γ平面源。为人体头颅等效模型剂量分布的快速计算提供了较为可靠的平面源。

大豆胞囊线虫抗性研究现状及其对大豆育种的启示 Review

大豆胞囊线虫(SCN,Heterodera glycines)是危害全球大豆生产最严重的线虫。作物轮作和培育抗线虫品种是防治线虫的主要方法。随着越来越多能够克服常用抗源的毒性SCN 群体出现,有必要寻找更多新的抗源或拓宽抗性遗传背景。本文综述了大豆抗SCN 基因的研究进展,以及这些基因如何相互作用,从而使大豆能够抗SCN。本文还提供了最新分子作图和分子标记,可用于不同抗性品系和品种的大规模选择和鉴定,以加速常规育种计划。深入了解SCN 寄生蛋白和大豆对病原体的抗性是通过基因修饰、基因叠加、回交或基因工程引入新的抗源而使抗源多样化的关键。

Towards Cr(VI)-free anodization of aluminum alloys for aerospace adhesive bonding applications: A review

摘要: For more than six decades, chromic acid anodizing (CAA) has been the central process in the surface pre-treatment of aluminum for adhesively bonded aircraft structures. Unfortunately, this electrolyte contains hexavalent chromium (Cr(VI)), a compound known for its toxicity and carcinogenic properties. To comply with the new strict international regulations, the Cr(VI)-era will soon have to come to an end. Anodizing aluminum in acid electrolytes produces a self-ordered porous oxide layer. Although different acids can be used to create this type of structure, the excellent adhesion and corrosion resistance that is currently achieved by the complete Cr(VI)-based process is not easily matched. This paper provides a critical overview and appraisal of proposed alternatives to CAA, including combinations of multiple anodizing steps, pre- and post anodizing treatments. The work is presented in terms of the modifications to the oxide properties, such as morphological features (e.g., pore size, barrier layer thickness) and surface chemistry, in order to evaluate the link between fundamental principles of adhesion and bond performance.

Scrutiny of non-linear differential equations Euler-Bernoulli beam with large rotational deviation by AGM

The kinematic assumptions upon which the Euler-Bernoulli beam theory is founded allow it to be extended to more advanced analysis. Simple superposition allows for three-dimensional transverse loading. Using alternative constitutive equations can allow for viscoelastic or plastic beam deformation. Euler-Bernoulli beam theory can also be extended to the analysis of curved beams, beam buckling, composite beams and geometrically nonlinear beam deflection. In this study, solving the nonlinear differential equation governing the calculation of the large rotation deviation of the beam (or column) has been discussed. Previously to calculate the rotational deviation of the beam, the assumption is made that the angular deviation of the beam is small. By considering the small slope in the linearization of the governing differential equation, the solving is easy. The result of this simplification in some cases will lead to an excessive error. In this paper nonlinear differential equations governing on this system are solved analytically by Akbari-Ganji’s method (AGM). Moreover, in AGM by solving a set of algebraic equations, complicated nonlinear equations can easily be solved and without any mathematical operations such as integration solving. The solution of the problem can be obtained very simply and easily. Furthermore, to enhance the accuracy of the results, the Taylor expansion is not needed in most cases via AGM manner. Also, comparisons are made between AGM and numerical method (Runge-Kutta 4th). The results reveal that this method is very effective and simple, and can be applied for other nonlinear problems.

Enhancing power transfer capability through flexible AC transmission system devices: a review

摘要: Global demand for power has significantly increased, but power generation and transmission capacities have not increased proportionally with this demand. As a result, power consumers suffer from various problems, such as voltage and frequency instability and power quality issues. To overcome these problems, the capacity for available power transfer of a transmission network should be enhanced. Researchers worldwide have addressed this issue by using flexible AC transmission system (FACTS) devices. We have conducted a comprehensive review of how FACTS controllers are used to enhance the available transfer capability (ATC) and power transfer capability (PTC) of power system networks. This review includes a discussion of the classification of different FACTS devices according to different factors. The popularity and applications of these devices are discussed together with relevant statistics. The operating principles of six major FACTS devices and their application in increasing ATC and PTC are also presented. Finally, we evaluate the performance of FACTS devices in ATC and PTC improvement with respect to different control algorithms.

Combined heat and power plant integrated with mobilized thermal energy storage (M-TES) system

摘要: Energy consumption for space and tap water heating in residential and service sectors accounts for one third of the total energy utilization in Sweden. District heating (DH) is used to supply heat to areas with high energy demand. However, there are still detached houses and sparse areas that are not connected to a DH network. In such areas, electrical heating or oil/pellet boilers are used to meet the heat demand. Extending the existing DH network to those spare areas is not economically feasible because of the small heat demand and the large investment required for the expansion. The mobilized thermal energy storage (M-TES) system is an alternative source of heat for detached buildings or sparse areas using industrial heat. In this paper, the integration of a combined heat and power (CHP) plant and an M-TES system is analyzed. Furthermore, the impacts of four options of the integrated system are discussed, including the power and heat output in the CHP plant. The performance of the M-TES system is likewise discussed.

Velocity gradient elasticity for nonlinear vibration of carbon nanotube resonators

摘要: In this study, for the first time, we investigate the nonlocality superimposed to the size effects on the nonlinear dynamics of an electrically actuated single-walled carbon-nanotube-based resonator. We undertake two models to capture the nanostructure nonlocal size effects: the strain and the velocity gradient theories. We use a reduced-order model based on the differential quadrature method (DQM) to discretize the governing nonlinear equation of motion and acquire a discretized-parameter nonlinear model of the system. The structural nonlinear behavior of the system assuming both strain and velocity gradient theories is investigated using the discretized model. The results suggest that nonlocal and size effects should not be neglected because they improve the prediction of corresponding dynamic amplitudes and, most importantly, the critical resonant frequencies of such nanoresonators. Neglecting these effects may impose a considerable source of error, which can be amended using more accurate modeling techniques.
